Summary:


Under the direction of the Clinical Manager of Inpatient Rehabilitation Services the Rehabilitation Liaison performs off-site preadmission screenings promotes the services of Lifespans inpatient rehabilitation facilities and facilitates appropriate admission to the departments.

Responsibilities:

CommunicationDemonstrates the highest level of communication and professionalism with patients caregivers rehabilitation staff providers and other health care professionals both within the academic medical center and in the community. Exhibits effective and professional verbal and non-verbal communication Communicates clearly respectfully and honestly with patients and/or caregivers and colleaguesCommunicates proactively with other caregivers involved in patientscare Adjusts communication and response to the level of understanding of the listenerCommunicates status of pre-admission assessments with medical directors and coordinates admissions with rehabilitation unit leadershipResolves problems and concerns by serving as a liaison between referral sources and Lifespan rehabilitation staff to improve service to the patient and the referring facilityCommunicates patient update and admission status to referring individual in a timely mannerInforms potential and accepted patients and their caregivers about Vanderbilt Rehabilitation Centers program and gathers and responds to input regarding their need for rehab informationInforms Lifespan rehabilitation leadership of issues and/or concerns in a timely mannerMarketingPromotes services of Lifespan rehabilitation programs to referring facilities discharge planners social service agencies physicians and third-party payersProvides educational programs for referral sourcesConducts/arranges tours of center for referral sources/family membersPre-Admission AssessmentConsiders age specific needs according to scope of services when planning implementing and evaluating careIdentifies and demonstrates ability to assess patients status consider any special needs and interpret appropriate rehabilitation and medical needs in order to make appropriate recommendation for IRF level of care to rehab physicianScreens all referrals for appropriate placement based on Medicare and other third-party payers guidelines and on the admission criteria established by Lifespan rehabilitation leadershipFinancialDemonstrates knowledge of current rules and regulations related to third party reimbursement and their impact on rehabilitation admissionsConfers with financial counselors on pre-admission financial screeningObtains pre-authorization for admission from third party payers as requiredOther DutiesProvides demographic data and referral statistics on screening information to Lifespan rehabilitation leadership for trend analysisDemonstrates knowledge of UDS and IRF-PAI to assure regulatory complianceDisplays competency in Quality Indicator scoringParticipates in departmental and/or interdepartmental quality improvement activities

Other information:

BASIC KNOWLEDGE:Graduate from a school of nursing and licensed to practice as a Registered Nurse in the State of Rhode Island . BSN and certification in rehabilitation nursing desired. Alternate credentials include license to practice Occupational Therapy Physical Therapy or Speech Language Pathology in Rhode Island. Knowledge of utilization review process health care reimbursement and physical rehabilitation principles.WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:Must be fit for duty in accordance with OSHA standards and program specific needs.
