JOB DESCRIPTION

An individual who provides assistance with activities of daily living and/or instrumental activities of daily living to consumers in accordance with the Service Authorization and/or Care Plan.

Essential Duties and Responsibilities:

• Provides assistance to assigned consumers including, but not limited to the following:

o Assistance with activities of daily living

o Assisting with ambulation and transfer activities and repositioning of client

o Assistance with nutritional and dietary needs

o Essential household duties

o Provide for safe and sanitary consumer environment

o Utilize basic infection control technique and procedures

o Provide medication reminders as directed by the Service Authorization and/or Care Plan

o Maintain consumer confidentiality

o Encourage consumer control of and promote the independent living philosophy

• Direct Care Worker shall deliver and document service to the consumer and document such service according to specific instructions from the Maxim designee and per Maxim policies and procedures. No changes in consumer care may be initiated without direction from Maxim designee. All changes in client status are to be reported to Maxim office

• Other duties as assigned

Minimum Requirements:

• Ability to read and write proficiently in English

• Successful completion of the Direct Care Worker Competency evaluation process

• Current CPR Card

• Current Health Screening (in accordance with state regulations)

• Provide proof of one of the following qualifications:

o A valid and verifiable Pennsylvania nurses license OR

o A valid and verifiable Pennsylvania certified nurse aide certification OR

o Certificate of completion from state approved home health aide program OR

o Has demonstrated competency by passing a competency evaluation developed by the Home Care Agency which meets the requirements of the state OR

o Completion of a training program developed by a Home Care Agency or Home Care Registry, or other entity which meets the requirements of the state or training program meeting the standards imposed on the agency or registry by virtue of the agency’s or registry’s participating as a provider in a Medicaid waiver or other publicly funded program providing home and community based services to qualifying consumers OR

o Successful completion of another program identified by the Department by subsequent publication in the Pennsylvania Bulletin or on the Departments website.

Compliance & Ethics Expectations:

• Participates and successfully completes the company’s compliance program requirements and adheres to the Code of Conduct, Company policies, and applicable federal and state requirements

• Sets an example for other employees regarding how the Company’s Code of Conduct and Compliance Program is applied and observed every day when dealing with customers, business operations, or other teammates

• Reports potential violations of company policy, Code of Conduct, and/or applicable laws and regulations to the company hotline, thorough the chain of command, to the Compliance and Ethics Department, or through other channels made available by the company for reporting potential violations

• Promotes an environment in which other employees are encouraged to report potential violations

• As appropriate, provides input and suggestions regarding areas in which policies, procedures, workflows, and/or controls can be improved to enhance compliance
