Overview
Under the direction of a professional nurse, the Home Hospice Aide assists the patient with personal care activities and incidental duties, traveling to their homes and long-term care facilities.. The Home Hospice Aide understands hospice philosophy and has the ability to comfortably provide specialized care to the terminally ill. The Home Hospice aide demonstrates competent knowledge and skills to assure care is appropriate to patients served:
Essential Functions:
Provides and assists with personal care to patients at their home according to established standards to maximize patient comfort. Assesses the patient’s temperature, pulse, respirations, and blood pressure to monitor his/her condition. Assists patients with prescribed exercises that the patient and Hospice Aide have had instruction by Registered Nurse or Physical Therapist to facilitate patient benefit from exercise program. Assists patient with application of TED/JOBST stockings. Observes patient’s condition and reports changes in condition to the Registered Nurse or Supervisor. Assists with patient transfers, ambulation, and prescribed exercises according to established procedures. Documents care provided and other pertinent observations in the clinical record to promote continuity of care. Completes patient assignment within acceptable timeframe and meets productivity standards. Effectively cares for the dying patient/patient’s family. Assists with light housekeeping tasks as indicated by HHA Plan of Care.Grand View Health Hospice patients reside both in home as well as in Long Term Care settings within a 15 mile radius of Grand View Hospital. Mileage is reimbursed.
Schedule: Monday- Friday, 8-hour day shifts. Every 4th weekend or less, depending on patient scheduling.
Qualifications
Education and Experience:
High School Diploma/GED, required
Qualifications:
Basic Life Support (BLS), Within 2 weeks of hire, required.
Valid PA Driver’s License and Auto Insurance, required.
Certified Nursing Assistant (CNA)
If not a certified CNA, 2 years’ experience in hospital, extended care facility, home care or Hospice after completion of 75 hours of supervised practical training under a suitable approved setting. On-site training program available for interested applicants.
Nursing students completing one academic year inclusive of direct patient care will be considered if certification requirements are fulfilled.
