Position Summary

The Physician Assistant (PA) or Nurse Practitioner (NP) will join the Hypertrophic Cardiomyopathy (HCM) Center in this fast-growing and exciting field of cardiology. The primary focus will be to see patients in an outpatient setting. The PA/NP will assess patients in the clinic to evaluate HCM and general cardiovascular disease and collaborate with physicians to implement the appropriate treatment plan. The PA/NP will also assume a leadership role in developing patient volume, educating potential referring doctors regarding the team’s capabilities, and providing in-service education to clinical providers and extenders.

Responsibilities

Perform chart review prior to clinic days and assist in creating a patient summary. Examine patients, complete medical history and summarize each patient’s condition to the physician. Educate patients and their families on conditions and treatment options including risks. Coordinate pre- and post-procedural testing in collaboration with the clinical coordinator. Review results of pre-and post-procedural testing and communicate results to patients, families, and referring physicians. Respond to patient and family inquiries. Assist patients in navigating through evaluation, procedure, and post-procedural follow-up. See both new patients and follow-up patients in the clinic and in other ambulatory settings. Address Telemed messages and inquiries from the front desk by the close of each day. Participate in coordination of care meetings. Help develop referral pathways and perform outreach to referring clinicians. Round on inpatients as needed. Perform other activities as assigned or as defined by patient need.

Minimum Qualifications

Bachelor's degree from an accredited Physician Assistant program or Master’s degree in Nursing, plus one to two years of related experience Current New York State PA or NP license Current NP or PA board certification Eligible for BCLS, ACLS, DEA, and NPI certifications Prior cardiology experience

Preferred Qualifications

Experience in lifestyle, nutritional, and medicational interventions for obesity. Prior clinical care coordinator experience. Familiarity with CPT and ICD-10 codes. Proficient in phlebotomy.
