Acute LVN

Duties include providing routine care, general observation of client’s health status, assists the RN and physician in treatments and communicating with clients and their families/guardians. Reports changes in condition and/or additional client needs to supervisor.

Responsibilities:

  • Administers medications and treatments as directed. Responds to emergencies and provides care.
  • Directs Certified Nursing Assistants in performance of work.
  • Maintains medical records as required.
  • Provides direct care, reorders residents' medications, charts summaries, and other pertinent information communicates status during shift change, checks supplies, and maintains equipment.
  • Reports all sudden changes to the physician immediately.
  • Assists in planning resident care activities based upon the Service Plan.
  • Participates in the interdisciplinary team.
  • Functions as shift Charge Nurse as assigned.
