Certified Peer Specialist-Case Manager

The Permanent Supportive Housing Program is for Fulton County residents inclusive of primary tenants and family. The Permanent Supportive Housing Program utilizes the Housing First and Harm Reduction models, ensuring a cost-effective way to help people with disabilities live more stable, productive lives. Supportive housing is widely believed to work well for those who face the most complex challenges. For those individuals who are homeless, have very low to no income, and/or have serious, persistent issues that may include substance abuse, mental illness, HIV/AIDS, or other serious challenges to a successful life, permanent supportive housing is a key to success.

River Edge is actively seeking a full-time Certified Peer Specialist Case Manager. The Certified Peer Specialist Case Manager will play a crucial role in providing support to individuals on their recovery journey. Responsibilities include leveraging personal lived experience to connect with and empower clients, assisting in the development of recovery plans, and offering guidance in navigating resources and services. This role involves collaborating with a multidisciplinary team, advocating for clients, and promoting a supportive environment. The Certified Peer Specialist Case Manager will contribute to the overall well-being of individuals by fostering a peer-supported approach to recovery. Duties and Responsibilities:
  • Collaborate with each client to develop and strengthen their Individual and Housing support plans according to individual goals and objectives using a person-centered approach.
  • Provide clients training in the areas of personal hygiene, accessing community resources, medical adherence, household management, or other needs as outlined in the client's Individual and Housing Support plan.
  • Perform home visits and off-site case management meetings.
  • Provide referrals for comprehensive services to ensure housing sustainability. 
  • Assist residents with the transition from homelessness by coordinating with emergency shelter and outreach services.
  • Alongside a Master Level Clinician, maintain a caseload of 18-20 clients.
  • Maintain contact with all housing clients on a scheduled basis that is appropriate to their service level and needs.
  • Identifies and provides intervention for potential crisis situations.
  • Assist with securing supportive services, including mental health, primary healthcare, and substance abuse support.
  • Facilitate relationship-building between landlords, caretakers, and clients. 
  • Provide transportation to appropriate community-based resources.
  • Communicate and collaborate with outside agencies and referral resources, including food stamps, Social Security, and disability benefits.
  • Enter participant information into the database in a timely matter and update as needed.
  • Completes client entries, exits, and self-sufficiency matrices.
  • Maintain complete and organized files.
  • Maintain and provide documentation on program goals and outcomes.
  • Maintain appropriate case documentation and client confidentiality.
  • Prepare and submit reports to the Supervisor or Team Lead as required.
  • Other assigned duties.

Qualifications:
  • A valid Peer Support certification (CPS) OR
  • Bachelor’s degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field. Certification is required or must be obtained within 12 months of employment.
  • 2-3 years experience working in community-based case management preferred.
  • 1-2 years experience working in a permanent supportive housing program preferred.
  • Proficient knowledge of Housing First and Harm Reduction models is preferred.
  • Candidates must have a primary diagnosis of mental illness or a dual diagnosis of mental illness and addictive disease, and a strong desire to identify themselves as a person with lived experience. The candidate has demonstrated experience in leadership, advocacy, or governance, and is well grounded in their recovery (one year between diagnosis and application to the training).
  • Must possess a valid Georgia Driver's License.
  • The applicant will have attended Georgia Department of Behavioral Health and Developmental Disabilities Certified Peer Specialist training and pass the Certified Peer Specialist certification process.

  • Communication: Strong verbal and non-verbal communication skills to facilitate meaningful connections with peers and convey information clearly.
  • Case Management: Proficiency in organizing and managing client cases, including coordinating services, maintaining records, and tracking progress.
  • Crisis Intervention: Skill in providing immediate support and assistance to individuals experiencing mental health crises, ensuring their safety and well-being.
  • Documentation: Ability to accurately record client interactions, progress, and treatment plans in accordance with organizational standards and confidentiality protocols.
