Overview

The Director of Health and Wellness Centers oversees both the administrative and support operations of Health and Wellness Centers in the Central Health system where clinical operations are provided by CommUnityCare as well as community wellness programs provided by Central Health. Currently, sites include the Southeast Health and Wellness Center, Northeast Health Resource Center, Hornsby Bend Health and Wellness Center. Additional health and wellness centers are under construction including Del Valle and Colony Park, that are under the direction of this role. This position is critical in working with others within Central Health and Enterprise in developing and implementing the organizational strategy around addressing population health. As we work to
better understand our role in non-medical drivers of health, this role is a contributor and directs the execution where appropriate.

The ideal candidate is someone who has worked in a health care setting to improve the health of disenfranchised
communities, understands the resource landscape locally, and can work collaboratively to address the needs of underserved populations in our service area. Development of community wellness initiatives including responsibility for programmatic infrastructure, implementation of contracts, processes, and procedures relevant to managing community wellness programs and the facility. She or he may be asked to evaluate the effectiveness and accountability of individual initiatives and partner programs. This role also assists in the community engagement to build a cooperative culture with key partners, patients, and government agencies to develop community programs in support of establishing the Health and Wellness Centers.

The Director of Health and Wellness Centers is responsible for coordination with the Clinical Operations Teams, Facilities, Facilities Management, and others for the administrative needs of the site. This includes preparing budgets, coordinating services, and overseeing the administrative teams on site. This position involves using a broad range of skills to provide accountable, operational leadership, oversight, and management of the facility and the portfolio of programs within health and wellness centers, which includes prevention services, resource navigation, and linkages of care. The director interacts with internal team members throughout the Enterprise and external partners, including clinical teams, community groups, consultants, community-based partners, government representatives, and the general community.

Responsibilities

Essential Functions:

Works collaboratively with CUC and clinical operations, Facilities and Facilities Management, and other internal stakeholders. Develops and executes community wellness programs based on data and in alignment with system strategy. Establishes effective working relationships with internal and external partners. Manages teams of community resource associates, administrative support associates, and program manager. Develops training and relevant protocols, processes, and procedures as new health and wellness centers come online. Works with CUC Practice leaders, CMO, and staff in coordinating IPE and GME activities as needed. Develops a budget and ensure the budget is adhered to throughout the fiscal year. Finds channels to hear from community members, patients, and stakeholders to constantly improve our delivery. Attends community events and meetings to promote the health and wellness centers. Assists in the design and delivery of new health and wellness centers. Works independently and exercises wise judgement when there are no rules or protocols, always putting Mission and Ethics first.

Knowledge, Skills and Abilities:

Preferred: Bilingual (Spanish/English) or some proficiency in Spanish.  Strong communication and presentation skills for internal and external audiences.  Cultivate effective community partnerships that advance population health.  Experience in staff training, facilities management, process improvement, change management, people management, and strategic planning Ability to lead meetings, work independently, prioritize and handle multiple deadlines, and work independently or in a team environment Manages departmental space, including assurance of appropriate space allocation and use, assessment of current and future needs and compliance with regulatory guidelines, in collaboration with the Facilities team. Expertly skilled in collaborating with internal and external partners to enhance community wellness, patient access and outcomes. Outstanding analysis skills, both quantitative and qualitative Highly responsive to internal and external stakeholders to develop and maintain strong relationships.

Qualifications

Minimum Education: 

- Bachelor's Degree

Minimum Experience: 

- 5 years Health care setting serving disenfranchised populations, population health, clinical administration, working with community based wellness programs, or in safety net health care systems.
