Job Summary: Provides quality diabetes self-management training to clients through assessing needs and developing, implementing, and assessing diabetes and other disease state outcomes.

Minimum Education: Bachelor's Degree as a Registered Nurse from a state approved program or Bachelor's Degree in Nutrition/Dietitian.

Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License, or State of Oklahoma Licensure as a Dietitian, and hold Certification as a Diabetes Educator. (CDE) or CDE eligible.

Work Experience: 1 - 2 years related experience.

Knowledge, Skills and Abilities: Knowledge of Microsoft Word, Publisher, PowerPoint; ability to learn PC applications.

Essential Functions and Responsibilities: Assists in the planning, organization, implementation, and evaluation of the nutrition portion of group and individual diabetes self- management training programs and consults. Develops lesson plans for group classes and outcomes for individual and group programs and consults. Assists in the planning, implementation, and evaluation of disease management protocols. Participates in diabetes related community programs as needed. Evaluates and develops appropriate nutrition education materials based on the needs of the patient population. Maintains accurate records for use in tracking patient outcomes. Actively pursues continuing education to improve nutrition and diabetes knowledge and skills. Develops an annual plan and professional goals. Adheres to professional code of ethics. Assists in providing non-nutritional related diabetes training.

Decision Making: The carrying out of non-routine procedures under constantly changing conditions, in conformance with general instructions from supervisor.

Working Relationship: Works directly with patients and/or customers. Works with internal customers via telephone or face to face interaction. Works with external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above.
