Discharge Planner (Social Services/Therapy Department)
Education/Training
Bachelor’s degree in Social Work or related field. Licensed if applicable in the state in which the hospital resides. BLS (CPR) Certified through AHA, American Red Cross or NSHI. Crisis Prevention Intervention Certification (CPI). Must be able to read, write, and communicate effectively in English.Experience Preferred: Two or more years of related Social Work or Behavioral Health experience.
Knowledge, Skills, and Abilities:
Excellent clinical and assessment skills. Knowledge of the treatment planning process. Knowledge of payment/reimbursement systems. Knowledge of patient rights and commitment to patient advocacy. Ability to collect, document, and analyze data. Understanding of claims submission process. Word processing and data entry skills. Network access to community, state, and national resources and health services organizations.Discharge Planning
Impact complex systems of care through coordination efforts to ensure timely placement of patients who have stabilized at the inpatient level of care. Conduct outreach and education with hospital clinical teams and outpatient providers to facilitate and ensure the development of timely and appropriate discharge plans. Appropriately document discharge planning outcomes and barriers; help identify and interpret any trends including systemic issues, access or placement needs and recidivism, and implement interventions to impact negative trends. Assist with accessing current medical information as needed. Facilitate access to behavioral health services, including assistance with locating providers and scheduling appointments as necessary. Coordinate the sharing of health information between providers and other programs. Document all interactions and coordination efforts with both internal and external customers in the care. Continuously self regulates professional presentation especially during emotional charged situations. Presents informed and available to medical staff during patient rounds or telephonic consultations. Promotes optimal outcomes for the patient within the boundaries of the diagnosis. Meets with patient, family, and IDT to facilitate appropriate discharge plans. Coordinates the discharge plan with the IDT and providers.Advocacy
Proactively collaborates with the health care team, payors, community agencies, providers and legal representatives to ensure appropriate discharge plan. Participates in care conferences, family conferences, etc. when indicated. Educates and supports the individual/family to be empowered and self-reliant in being advocates for themselves. Represents the individual’s best interests through assisting in finding appropriate discharge placement.Monitoring & Evaluation
Continuously reviews and evaluates the patient’s progress, as reflected by the goals defined in the treatment plan. Uses appropriate auditing processes and tools to ensure department and treatment efficiency. Participates in performance improvement evaluation processes with particular emphasis on results-oriented treatment.
