Position: Adult-Gerontology Nurse Practitioner (AGNP)
Type: Full-Time/Part-Time
Responsibilities:
- Provide advanced clinical care to adult and geriatric patients, including assessment, diagnosis, and treatment of acute and chronic conditions.
- Develop and implement personalized care plans in collaboration with patients and their families.
- Perform comprehensive physical examinations and order diagnostic tests as needed.
- Prescribe medications and manage treatment regimens according to best practices and guidelines.
- Educate patients and their families about health management, disease prevention, and lifestyle modifications.
- Collaborate with interdisciplinary healthcare teams to ensure coordinated and effective patient care.
- Document patient interactions, treatment plans, and outcomes accurately and thoroughly.
- Stay current with advancements in gerontology and best practices in patient care.
Qualifications:
- Master’s degree in Nursing (MSN) with specialization as an Adult-Gerontology Nurse Practitioner.
- Valid state certification and licensure as a Nurse Practitioner (AGNP).
- Experience in a clinical setting, preferably with a focus on adult and geriatric care.
- Strong diagnostic, clinical, and communication skills.
- Compassionate, patient-centered approach with a commitment to quality care.
- Ability to work independently and as part of a multidisciplinary team.
