Clarity on the Role

The Supervisor, Care Management (Part-Time), is responsible for supervising care management operations and supporting a team of approximately 5-10 team members to deliver services that engage, motivate, and educate family caregivers to support complex individuals, keeping care at home. This individual ensures the delivery of high-quality services in compliance with all state, program, and accreditation requirements. This individual will support a team of clinical care managers, providing care management services across the continuum of care (assessment, care planning, care coordination, and monitoring and evaluating services). In collaboration with key stakeholders, this individual will create action plans in support of operational enhancements, customer satisfaction, quality improvement efforts and positive health outcomes.?

What's Expected: 

Supervise a team of clinical care managers (approximately 5-10 team members) to engage, motivate, and educate family caregivers to support complex individuals, transitioning from inpatient stays to next site of care, working with practices to ensure communication is bi-directional between provider and care manager and keeping care at home centric to the care management process when possible.? Responsible for daily administrative tasks – including; team & individual performance reporting, quality assurance oversight, resolution of workflow barriers & blockers, and internal & external communication of operational updates. Track and trend outcomes for potential improvements in the care management process. Create monitors, evaluate, and disseminate programmatic reports weekly, monthly, and as needed? Plan, organize, and oversee timely completion of activities, including assessments, care plans, and discharge planning? Implement and interpret organizational and contractual policies and procedures; regularly review work to ensure policies and guidelines are appropriately applied Identify and support the implementation workflow enhancements to improve patient care quality and operational efficiency. Support the team’s Manager in overseeing care management functions and ensure consistency and compliance with clinical policy, procedure, and best practices, including regulatory and contractual agreements.? Familiarity with NCQA accreditation requirements? Coach and develop staff to meet department goals, ensuring key performance indicators and goals are reviewed regularly in support of a high performance / high engagement work environment, with support from the team’s Manager. Provide oversight for the appropriate use of tools and person-centered care plans to support caregiver and care recipient needs; provide regular review of incidents to identify cause and recommend interventions? Support and maintain clinical relationships with key external business partners and referral sources to increase care collaboration and foster credibility in our services? Perform other duties and special projects as assigned?  Requirements Integrity: The right way is the only way. Education (Degree/Major):

Bachelor’s degree or equivalent, preferably in nursing health care is required

Certifications:

An active RN Compact state license? Additional non-compact state-specific qualifications may be required 

Years of Related Experience:

7+ years of care management experience in healthcare, preferably with experience in eldercare, and/or people with disabilities or other complex medical needs? 3+ years of experience managing, coaching, or developing team 2+ years of program or clinical operations management?

Skills:

Experience working with Health Systems required Located in the Orlando area is preferred Experience dealing with enterprise scale EHR administrative work is required Stellar people leading skills and motivational engagement? Ability to travel to corporate meetings if needed? Working knowledge of care management systems, community service and delivery systems, and proficiency with Microsoft Office suite
