What will you do in this role…

Utilize Upstream’s Telehealth platform (Raintree Connect) to provide Occupational Therapy telehealth services to patients Conduct virtual evaluations (and re-evaluations) to develop a personalized treatment plan with measurable goals and alter as needed Collaborate with patients, caregivers, and other healthcare professionals to ensure effective communication and support (monitor patient compliance) Guide and instruct patients on therapeutic exercises and activities to improve mobility, strength, and function Maintain accurate and updated patient documentation and records

Qualifications

Graduate of an accredited Occupational Therapy program Current or pending state licensure Minimum 2 years of experience as an Occupational Therapist Cognitive therapy and outpatient experience preferred Reliable high-speed internet connection and a dedicated workspace for virtual sessions
