WHAT YOU CAN EXPECT

Participate in people management activities for direct team members such as conducting performance evaluations, disciplinary actions, and interviews.

Participate in performance improvement activities.
-
Provide education to the beneficiary, support system, and/or the healthcare team regarding disease management and evidence-based practice guidelines with appropriate tools.
-
Document and communicate a change in health status to key individuals such as PCP and clinicians providing current/recent care.
-
Perform appropriate interventions and document to meet the needs of the beneficiary, their family, and the healthcare team based on scientific principles, theoretical knowledge, and clinical expertise.

Utilize motivational and health-coaching techniques to engage the beneficiary and support system.

Assess, intervene and interview beneficiaries as needed when consultation requested by the provider, Clinical Case Coordinators, Manager-Case Management, other clinicians, others.

Determine the effectiveness of the plan of care through beneficiary assessment and document Healthcare outcomes.

Provide or facilitate appropriate education, using a beneficiary and family-centered approach, to facilitate the achievement of health goals.

Reassess and modify the plan as necessary to achieve health goals.
-
Understand and apply federal and state regulations effectively. Demonstrate knowledge of ACO/ICM organizational policies.

Maintain a seamless and coordinated acute/post-acute care continuum for ACO patients discharged from Franciscan Health facilities systemwide.
QUALIFICATIONS

Bachelor's Degree in Nursing required
-
Active Indiana RN license required
-
5 years of hospital/managed care experience required
-
5 years of current nursing experience required
-
2 years of case management/coordination experience required

Basic Life Support Program (BLS) & Cardiopulmonary Resuscitation (CPR) - American Heart Association required
-
Certified Case Manager (CCM) or Commission for Case Manager Certification (CCMC) strongly preferred

Prior leadership experience preferred

Experience with hospital information systems (e.g, order entry, results reporting) preferred
