The Home Care RN provides to patients in the home setting utilizing the nursing process of assessment, planning, implementation and evaluation of safe and therapeutic care. Responsibilities:

· Ensures quality nursing care is rendered to all.

· Utilizes the Nursing Process, plans, directs and supervises the care of patients in addition to being involved in the provision of direct care of patients and families.

· Develops a written plan of care that guides nursing intervention and facilitates desired outcomes.

· Implements interventions based on the plan of care in accordance with client and caregiver level of skill and knowledge.

· Evaluates client/ family responses to interventions and changes the plan as required.

· Document care provided and client/ family responses consistent to agency policy and procedure.

· Follows agency policies and procedures.

· Communicates with physician and other disciplines regarding client status. Coordinates services as needed.

· Utilizes community resources in the development of plan of care.

· Supervises ancillary personnel (Home Health Aide, Shift Professional Nurse) assigned to client care.

· Attends educational programs to maintain current standards of practice.

· Responsible for employee injury prevention, follows safety rules, and addresses safety issues with supervisor.

Qualifications:

· Must be graduates from an accredited School of Nursing with current NYS RN license.

· 2 years of nursing experience preferred.

· Home Care experience preferred.

· Strong verbal and written communication are necessary and basic computer skills are preferred.

· Must possess valid NYS driver’s license and current automobile insurance and able to drive during work hours.
