Job Description Oncology Pharmacist Rx relief is seeking an Oncology Pharmacist to work in a oncology pharmacy setting in the San Rafael, CA area. We are looking to fill this positions quickly Pay / Benefits / FAQs for the Oncology Pharmacist positions: Pay: $70.00 - $80.00 per hour - Depending on experience Benefits: Employee resources : FAQs : Oncology Pharmacist Job Specifics and Requirements: Must be able to work in the San Rafael, CA area Must have a valid, clear and active California Pharmacist license B.S. or Pharm. D. 2 years of oncology experience as a licensed Pharmacist Background and Drug Screening Health Screening required Oncology Pharmacist Responsibilities: Responsible for oncology patients, you must work independently and effectively to ensure appropriate and accurate drug reviews. Clinical focus will be in hematology/oncology working closely with the physician team in monitoring patients on chemotherapy. Interpret prescriptions and/or physician orders for accuracy, eligibility, availability, cost effectiveness and therapeutic appropriateness. Review patient medication profiles, contact physicians and make authorized changes to orders or prescriptions, as required. Provide drug information on medication use, dosage, side effects and composition to physicians, nurses and patients/families. Partner with other healthcare professionals to develop evidence-based clinical pathways and/or treatment protocols. Conduct utilization reviews and identify opportunities to optimize medication use for adult and pediatric oncology patients. Generate and analyze process and outcome measures for use in assessing oncology-related patient outcomes; report impact of quality programs. In conjunction with the Investigational Drug Trials Coordinator, assist in the coordination of oncology investigational drug services. Communicate directly with patients and their families on all aspects related to chemotherapy therapy, promoting high levels of knowledge, engagement, and adherence. Supervise and mentor other pharmacy team members, and other healthcare professionals to enhance clinical competency on topics related to chemotherapy. Demonstrate knowledge of oncology protocols, sterile compounding, USP 797/800 workflows and Pharmacy State Law. Responsible for verifying orders per protocols. Independently design, secure funding for and conduct clinical research within the scope of oncology practice.
