Community Health Nutritionist
Summary:
The Community Health Nutritionist provides comprehensive nutrition education services to the Forest County Potawatomi Community through educational programs and office/home visits with an emphasis in the area of nutrition and wellness.
Duties and Responsibilities:
- Works with the Registered Dietitian on the planning, implementation, monitoring and evaluation of Forest County Potawatomi Nutrition Services including but not limited to: Pre-School Nutrition Education, Summer Lunch Program, Day Camp activities, Food Handling Safety, Community Events including Spring and Fall 5Ks, Cooking Classes and Demonstrations, Diabetes Program, Elderly Nutrition Program.
- Develop and provide prevention education and events or activities that will promote healthy life-style choices. Additionally, develops nutrition related articles, educational materials and other communications
- Provides nutrition services and education for wellness, prevention, and management of a variety of nutritional related conditions , and makes referrals to other agencies as needed
- Active participant on Nutrition and Physical Activity related coalitions and committees within and outside of the FCP Community representing the FCP Community Health Department
- Enter client specific data into official client medical record using the Electronic Health Record or other data collection systems as required
- Serves as a liaison between Medical/Community/Elderly Staff and other Tribal Programs on behalf of clients needing nutrition education
- Participates in Quality Improvement and Public Health Accreditation activities as requested
Requirements (Education, Experience, Knowledge, Skills, Abilities):
- Bachelors Degree in Dietetics, Nutrition, Health Education or Health Promotion/Wellness with a minor in Nutrition, CHES preferred
- Public Health or Tribal Community Health experience preferred
- CPR and Basic Life Support Certified
- Complete and maintain required practice hours and continuing education credits per CDR and WIC Requirements
- Must have and maintain a valid drivers license and be legible for coverage under the Tribes insurance
- Must successfully pass all applicable background checks and drug screens
- FCP complies with all CDC vaccine and testing recommendations for healthcare professionals. Proof of immunizations is required and immunizations/testing provided upon hire.
