Physician Practice Triage Nurse

Summary
Triage Nurse will screen patient to determine the severity of condition and whether an appointment with the physician is necessary. They establish the needs of the patient and will help coordinate care as needed at both Primary Care Clinics.
Job Description
*Assess the health need of the patient and determine the level of care needed for the patient. The nurse will be responsible for recommending emergency care, home care or a doctor's appointment based on the patient's condition.
*Retrieve patient phone and portal messages and appropriately assigned messages.
*Coordinate continuity of care for the patient.
*Communicate and document knowledge of patient assessments.
*Any additional task as assigned by Practice Manager.
Requirements
•Graduate from an accredited school for Registered Nurses.
•Knowledge of current trends and techniques in nursing appropriate to practice setting.
•Current unencumbered RN licensed in the State of North Carolina.
Preferred
1- 2 years experience preferred.
