Function:

  • Receives, reviews and coordinates patient clinical referral activity
  • Completes admission assessments
  • Serve as Charge Nurse on short term rehab unit
  • Inputting physician orders in system
  • Assists with arrangements and confirmation of all admissions
  • Coordinates the assignment of attending physician
  • Conducts discharge assessments
  • Completes discharge packets
  • Conducts immunizations and covid testing for new residents
  • Promotes optimal communication among program staff members, facility staff, referrals sources, physiciansand patients
  • Adheres to all safety and risk management protocols and programs
  • Participates in direct and indirect patient care activities as warranted
  • Participates in quality improvement activities and QAPI process to increase efficiency through improved systems and processes
  • Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, administrators and coworkers
  • Performs other duties or functions as directed
Responsibilities:

This position handles review of clinical data on referrals, assesses new admits, completes EMR data entry.

Requirements:
  • High School graduate
  • If RN, must have a current license to practice profession in state
  • Must have a reliable source of transportation
  • Valid driver's license and proof of insurance
  • Ability to understand, remember and carry out oral and written instructions in English
  • Ability to work in Excel, Word, Power Point, PCC and Nexion dashboard or have the ability to learn
  • Ability to speak clearly and hear well enough to communicate with others
  • Ability to complete assignments while having constant interruptions
  • Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation
  • At least one year of office or LTC clerical experience
  • Knowledge of medical terminology preferred
  • Ability to type
  • Prior experience in admissions preferred
