General Summary:Through the Supportive Services for Veteran Families (SSVF) program, we help veteran families to secure permanent housing. The SSVF program works to maximize high-quality housing options and to access and manage the available financial resources to prevent and eliminate homelessness. The Shallow Subsidy (SS)service helps housing insecure veterans achieve housing stability through rental assistance and case management.The case manager is responsible for managing the needs of the individuals enrolled in Shallow Subsidy programming.The position requires excellent people skills, entrepreneurial mindset, and collaborative orientation. The case manager will work alongside the SS Housing Specialist and the SS Program Manager in order to ensure that all needs of the Veteran have been met. The Program service areas span Maryland.Principal Responsibilities and Duties:1. Community Engagement, Outreach and PartnershipConduct outreach activities to maximize opportunities for contact with persons who, without assistance, are likely to remain or become literally homeless.Collaborate and plan for equitable resource distribution to meet veteran household and community needsAssist and support the SS team by keeping communication active with veterans' organizations, housing organizations, veterans, and other community partners to educate about new services available under SSVFAssist to maintain relationships with landlords, property managers and housing providers to continue the supports in high-quality housing 2. Compliance, Quality Improvement, Evaluation and ReportingEnsure that monthly goals set forth by SS program manager and service line leadership are achievedEffectively prioritize and execute tasks in high pressure situationsDocumentation and procedures are followed to meet and ensure compliance with all governing entities3. Budgetary Responsibility and Fiscal Oversight Proactively manage referrals, intake, and case load to ensure spend-down that aligns with program goals, funder requirements, and organizational mission4. Case Management Manage a case load of veteran participants in the Shallow Subsidy program, providing Shallow Subsidy case management services to low-income Veteran participants consistently throughout a 2-year period.Develop, implement and revise 90-day and 2-year housing stability plan with Veteran participants.Ensure exceptional service to veteran clients, demonstrating compassion and responsivenessProvide onsite and off site case management supportsAid in daily living and social skillsProvide crisis intervention if necessaryEncourage the development of natural supportsAttend/participate in required staff and program meetingsKnowledge, Skills, and Abilities Required:Bachelor's degree, with 1- 3 years of related experience.Commitment to "Housing First" philosophy and strategies.Knowledge of housing, tenant rights, and related areas.Excellent oral, written communication and interpersonal skills.Ability to travel within the community, some rare weekend availabilityExperience with computers and knowledge of commonly used office software, e.g. Word, Excel, Outlook, Zoom, PowerPoint, WebEx.Ability to learn relevant technology and software platforms, including HMIS and electronic health recordsValid driver's license and a car.Strong written and verbal skillsExperience with chronic behavioral health issues and/or knowledge of substance use disorders preferred.Spanish speaking ability preferred.Working Conditions: Note: Reasonable accommodations may be made to enable a qualified individual with a disability(s) to perform the essential functions of this position.Normal office environment where there is no physical discomfort due to temperature, dust or noise.Work in community sites where homeless individuals gather where there may be potential exposure to communicable disease and/or minimal risk of physical injury.Depending on specific assignment, work requires travel between programs and services.#IND123
