On-Call Registered Nurse Remote

Self-Direct, Inc. provides high quality care to individuals in NYS who are at risk of nursing home placement, or who need extra care at home or in the community. Our goal is to help clients maintain their independence and live a safe and healthy lifestyle.

Self-Direct, Inc., NYS DOH Licensed Home Care agency is currently seeking a remote, per diem On-Call Registered Nurse to provide clinical telephonic triage for clients and home care support staff after normal operating hours and weekends.

About this position:

• Promptly triage clinical related client/caregiver phone calls, providing assessments, advice and instruction in accordance with Self-Direct, Inc. policy and procedure and within the scope of practice for RN's in the state of New York
• Report and record clinical notes, progress notes, changes in the client condition after telephonic contact
• Complete, maintain, and submit timely and accurate clinical related documentation
• Provide phone orientations, communicating all care plan information to home care support staff

Qualifications:

• Current NYS RN license
• Two (2) years of home care experience, preferred
• Strong interpersonal, verbal, and written communication skills required
• Computer literacy, including but not limited to data entry, retrieval, and report generation

Compensation: $75 to $150 per day
