General summary

Works independently and often with complex problems that relate to drug therapy. Provides drug information to Medical and Nursing staff. Has complete knowledge of all medication use processes including drug distribution, packaging and labeling and IV admixture. Ensures compliance with regulatory agencies and local, state and federal legislation.

Duties and responsibilities
  1. Dispenses medication to hospital inpatients and outpatients pursuant to valid prescribe orders and in accordance with hospital policies and procedures and applicable state and federal laws.
  2. Directly responsible for distribution, control and safe use of medications for hospital patients. Insures proper technique and accurate preparation of all pharmaceutical products including: oral medications, topical medications, IV admixtures, TPNs, investigational drugs, etc.
  3. Performs and documents monthly inspections the patient care area(s), checking for safe storage and control of drugs and chemicals.
  4. Supervises the work of pharmacy technicians and interns, if applicable. This includes, but is not limited to, setting or resetting work priorities, assuring proper staffing to accommodate workload, teaching and reinforcing proper procedures, and providing other direction and/or assistance as needed.
  5. Responsible for enforcement/compliance of hospital and department policies and procedures relating to medication use and control.
  6. Performs patient education activities and special projects as assigned.
  7. Reports suspected Adverse Drug Reactions, medication errors (Incident Report) and drug product quality problems according to departmental policies.
  8. Performs drug therapy monitoring activities, per approved hospital protocols (pharmacokinetics, renal dosing, antibiotic monitoring, etc.) as trained and assigned for patients of all ages.
  9. Provides answers (written or verbal) to drug information inquiries as they arise (to medical staff, hospital staff and patients) - using department references and computerized clinical information systems.
  10. Legibly documents dispensing activities, order clarifications, interventions, controlled substance issues and other department activity as appropriate. Enters interventions into the departmental intervention database as appropriate.
  11. Enforces the hospital formulary, encouraging the use of formulary agents where appropriate.
  12. Responds to internal and external disaster situations as defined in hospital policy.
  13. Participates in the evaluation process of support associates.
  14. Assists in the education and training of new pharmacists, pharmacy interns, pharmacy technicians, students or other medical associates as appropriate.
  15. Provides input to pharmacy management regarding ways to improve pharmacy operations.
  16. Maintains current Michigan pharmacist licensure. Obtains up-to-date knowledge of pharmaceutical items through appropriate educational activities in order to maintain licenses.
  17. Participates in the maintenance of adequate stock levels of department pharmaceuticals and supplies to ensure availability.
  18. Demonstrates in all duties and responsibilities a warm, humanistic attitude toward patients, their families, visitors and employees of the hospital.
  19. Other duties as assigned.
Educational requirements
  1. Bachelor of Science or Pharm.D. Degree required.
  2. State of Michigan Pharmacist and controlled substance licenses required.
Experience

Previous hospital pharmacy experience preferred.

Specific requirements
  1. Demonstrates high-level interpersonal skills to deal effectively with patients and hospital staff, including fostering teamwork.
  2. Demonstrates the ability to prioritize workload to meet patient needs, responding to emergency situations as required.
  3. Demonstrates a thorough knowledge of safe handling of antimicrobials, biologicals and antineoplastic agents.
  4. Demonstrates a thorough knowledge of aseptic technique and basic principles of infection control.
  5. Demonstrates a high level of analytical ability related to complex problem solving involving drug therapy, including drug interactions, drug-drug incompatibilities, adverse drug reactions and drug dosage ranges.
  6. Demonstrates a thorough knowledge of trade and generic names.
  7. Demonstrates computer keyboard familiarity and order entry skills, as well as light typing ability and ability to operate electronic dispensing equipment.
  8. Other physical activities required to perform essential job duties and responsibilities include walking or otherwise independently getting around, lifting, reaching, bending and stooping, standing, writing, hearing seeing, speaking English.
  9. Other mental processes required to perform essential job duties and responsibilities include observing, identifying, remembering, understanding and problem solving.
  10. Works in an area where antimicrobials and biologicals are prepared which provides for the possibility of accidental exposure.
