ResponsibilitiesThe primary function of the Patient Navigator-Oncology is to guide patients through the health care system by assisting with access to care issues, developing relationships with service providers, tracking interventions, tracking outcomes and follow-up. Educates patients and families on diagnosis and recommended treatment options throughout the continuum of care including surgical interventions, procedures, ancillary tests, and post discharge follow up. QualificationsRequired:Three years nursing in acute care settingBachelor Degree in NursingMust be a Registered Nurse (RN) and one of: Oncology Certified Nurse (OCN), Certified Nurse Perioperative Care (CNOR), Certified Asthma Educator (CAE), Certified Breast Care Nurse (CBCN), Certified Gastroenterology RN (CGRN), Certified Pediatric Hematology Oncology Nurse (CPHON), Certified Pediatric Oncology Nurse (CPON), Oncology Nurse Navigator-Certified Generalist (ONN-CG)
