POSITION SUMMARYThe Intake Coordinator, Nutrition and Behavioral Health Service is responsible for providing support to the Nutrition and Behavioral Health Services Departments. Duties will include assisting the Nutrition and Behavioral Health Teams with ensuring referrals are assigned and completed in a timely manner. The Intake Coordinator will ensure lines of communication remain open with the Nutrition and Behavioral Health Teams and the clinical staff seeking referrals for their patients. The Intake Coordinator will be responsible for assisting with program compliance, internal and external referrals, scheduling appointments and follow ups.The Intake Coordinator will interface with the Billing staff to ensure accuracy of documentation for clean claims and maximum reimbursement. The Intake Coordinator will assist the Medication Assisted Treatment (MAT) Coordinator providing support to the patients enrolled in the MAT program. ESSENTIAL FUNCTIONS: Assist Nutrition and Behavioral Health Services Teams with scheduling/re-scheduling appointments and incoming referrals for servicesFollow up in eCW to ensure Nutrition and Behavioral Health providers are receiving and completing referrals in a timely fashionTrack referrals and analyze data as requiredAssist the MAT Coordinator with managing internal and external referrals of patients requiring treatment.Communicate and collaborate with providers, nurses, dietitians, social workers, MAT coordinator and all members of the multidisciplinary health care team as needed to facilitate patient care.Monitor and document patient progress toward services plan and goals.Ensure completion of all required documentation for existing and new MAT patients.Act as secondary source for supporting the MAT team and grant tracking, data entry and required reporting.Maintain familiarity with program complianceAssist MAT coordinator with monthly clinical reports and accounting.Required Education and Experience:Bachelor's DegreeTwo years' experience administrative services and customer serviceRequired CompetenciesAccuracy - Ability to perform work accurately and thoroughlyCommunication - Ability to communicate effectively verbally and in writingComputer Skills - Proficient ability to use a computer, including Microsoft Office Suite and electronic medical recordConfidentiality - Maintain patient and employer confidentiality. Comply with all HIPAA regulationsCustomer Service Oriented - Friendly, cheerful and helpful to patients and others.
