Description Title: Clinical Pharmacist Location: Fountain Valley (Remote) Department: Pharmacy Status: Full Time Shift: Day - 8 Hrs Pay Range*: $ 72.31 /hr - $104.86/hr The Medication Therapy Management (MTM)/Complete Medication Management (CMM) Clinical Pharmacist is responsible for performing various clinical duties related to specific pharmacy-related quality metrics pertaining to adherence to therapy, appropriateness of therapy and medication reconciliation.This includes but not limited to reviewing and identifying population data gaps based on clinical criteria and specifications associated with pharmacy quality metrics. The Pharmacist will provide in depth clinical and utilization expertise to physicians and nurses for MemorialCare Medical Foundation (MCMF) and targeted telehealthoutreach to patients.The Pharmacist must have the demonstrated ability to work independently with minimal supervision. Essential Functions and Responsibilities of the Job Perform clinical and utilization review duties related to specific pharmacy-related quality metrics pertaining to adherence to therapy, appropriateness of therapy and medication reconciliation.This includes but not limited to reviewing and customizing patienttelehealth outreach based on clinical criteria and quality specifications. Determine/obtain resources needed to support clinical pharmacy services including but not limited to literature review, protocol development, and data collection. Provide drug information services to physicians. Consultwith physicians pertaining to questions and interpretations of clinical quality measures. Address patient questions and concerns related to medication therapy and adherence. Work collaboratively with the performance improvement department, health plans in joint strategic initiatives. Participate in provider and patient education on relevant clinical and quality updates. Recommends possible strategies to promote cost-effective therapy to patients and providers. Contribute to team or departmental projects directed toward the achievement of improved service efficiency, patient outcomes and quality scores. Determine/obtain resources needed to support implementation of quality initiatives for clinical pharmacy services including but not limited to literature review, protocol development, data collection, outcomes measurement and analysis. Perform targetedtelehealth patient outreach for comprehensive clinical medication review and optimizing outcomes. Possess strong working knowledge of pharmacy-related quality metrics and targeted goals. Provide drug information services to MCMF providers. Participate in departmental quality and performance improvement initiatives. Support implementation of initiatives for clinical pharmacy services. Possess strong analytic skills. Ability to follow company policies, procedures and directives. Ability to interact in a positive and constructive manner, p rioritize, multitask, and be at work on time. Other duties as assigned. Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Check out our MemorialCare Benefits for more information about our Benefits and Rewards.Qualifications Experience Minimum 2 years clinical pharmacy experience required Managed care and/or provider group experience preferred Effective verbal and written communication skills Demonstrated ability to exercise collaboration, negotiation and other interpersonal skills Comprehensive knowledge of pharmacology, medical terminology, pathologies, disease states and related information to medical management plan Ability to utilize analytical skills related to collection, synthesis, analysis and reporting of data Basic PC skills including Microsoft Word, PowerPoint and Excel required Education Doctor of Pharmacy Degree, from an accredited school of pharmacy required Successful completion of post graduate residency training preferred Current license to practice pharmacy by the California Board of Pharmacy
