The RN position is responsible for coordinating assessment, follow up and implementation of resident care.Principle Accountabilities / Essential Job Functions: To coordinate timely and accurate nursing assessments, interventions and evaluations with supporting documentation in the resident's record to assure that desired resident outcomes and standards of resident care are provided.Assist in planning, directing and providing resident care as a front line leader. This will include but not limited to medication administration, data collection, assessment, provision of treatments, risk assessments and implementation and care plan development.Recognize signs and symptoms of change of condition-respond as indicated by resident need and follow through with appropriate contacts to include clinical manager, family and MD.To schedule and coordinate follow-up assessments / interventions with other staff.Attendance of required staff meetings, daily report and continuing education sessions in order to be informed of current processes and plant to provide quality resident services.Must be able to work positively with team members and support all aspects of team approach.Orientation and training new staff as needed.Abide by all facility rules and regulations.Be able to work outside of normal working hours as necessary.Responsibilities: Performs all duties in a professional manner. Sets a good example for employees (timeliness, accuracy, appearance, demeanor, etc) Gains the trust and respect of co-workers and management, develops and maintains positive working relationships with them.Responsible for the quality, accuracy, thoroughness and reliability of work performed and for maintaining professional standards required by the position and by regulation in the long-term care industry.Maintains the integrity of confidential resident, family, employment and business information (including salaries).Is respectful of the right of residents, notifies supervisor of questionable care or mistreatment.Other duties as assigned.Observes Resident's Rights.QA knowledge and involvement.Qualifications/Education: Graduate of an accredited school of nursing and must possess and maintain a current Wisconsin license.Must be able to demonstrate competency/proficiency with computers and certain medical equipment.Requires ability to positively relate to the needs of nursing home clients and have a genuine appreciation and interest in older people and those with special needs.Must be able to speak, read and write English in a proficient, understandable manner.Must be physically able to perform all duties of the position.Must be able to make independent decisions in the best interest of the organization.Requires patience, tact and willingness to work with and communicate effectively with people regardless of the level at which they are currently functioning.Must be able to work effectively with he public under all conditions.Must meet the requirements of the criminal background check law.Requires regular and punctual attendance.
