Responsibilities will include:Initial client assessments to determine care needsDevelopment of individualized client care plansCase management including building and maintaining relationships with assigned clientsSupervisory field staff visitsPerform re-assessments and care plan updates as needed to maintain client's optimal statusCaregiver orientation to start of caseProvide in-person and on-call support to clients and caregivers as neededMaintain client recordsCaregiver competency assessments Requirements:- Valid NJ RN license- 1 year supervisory experience preferred- 1 year home care experience preferred
