Care Coordinator

SUMMARY OF RESPONSIBILITIES:
Engages clients in the recovery process. Supports the client’s recovery through a holistic approach that considers each client’s overall biopsychosocial needs and strengths. Coordinates community-based services to provide individualized and integrated care for individuals experiencing serious substance use disorders, or co-occurring substance use and mental health disorders. Assists individuals in navigating complex systems to address all identified needs.
MINIMUM QUALIFICATIONS:
- Bachelor’s Degree in a behavioral science or related field.
- Special Skills: Basic knowledge of substance abuse and mental health treatment; working knowledge of community resources; engagement, motivational, and advocacy skills; strong interpersonal/communication skills.
- Strong computer skills for electronic health record documentation.
- Identifies client strengths, needs, abilities, and challenges. Develops initial case management plan with client to address behavioral health issues as well as medical, social, housing, interpersonal problems/needs that impact the client’s status.
- Provides intensive support services, actively linking client to necessary resources, monitoring client participation in recovery process, navigating community-based services to address identified needs.
- Encourages Medication Assisted Treatment, when appropriate.
- Links providers of different services to enable shared information, joint planning efforts, and coordinated treatment and support services.
- Encourages self-directed recovery efforts.
- Ensures linkage with Recovery Peer Specialist for ongoing support of the client’s recovery process.
- Re-assesses client needs to prevent relapse and promote recovery and wellness.
- Advocates on behalf of client.
- Provides emergency/crisis intervention services when needed.
- Maintains electronic health, accurately completes required documentation within designated timeframes.
- Other duties as assigned by Senior Director of Central Receiving Services
