Our Hospice Team is looking for a Clinical Liaison who will be driven by the desire to directly impact the lives of those we serve, our community partnerships and each other. This role is a crucial pillar of clinical support to our world class sales team.

General responsibilities include:

    • Meeting and/or speaking with families in person and via telephone regarding Hospice Services. Example: explanation of benefits, informational visits, consents, etc.
    • Assisting Account Managers with community and patient education.
    • Position involves driving to different locations.
    • Guides patients, families, caregivers, community partners through the hospice philosophy, assessment and admissions process.
    • Work closely with VP of Growth and Development, Admissions Manager, Intake Coordinator, and Account Managers to understand the patient's needs and determine patient's eligibility for hospice care.
    • Utilizes knowledge to evaluate hospice eligibility per Medicare guidelines and obtain information to support the same.
    • Provides compassionate education concerning hospice care philosophy, as well as the nature of the care they can expect to receive in regard to their specific terminal illness.
Ideal background includes:
    • Licensed Vocational Nurse with current CA License
    • Hospice background
    • Relationship building skills, an entrepreneurial spirit, and the desire to build and support community outreach efforts
    • Knowledge and experience working with home health/hospice patients and families, either professionally or as a volunteer is required
    • Firm understanding of Hospice Criteria and community resources for candidates not quite appropriate for hospice
