This person will work closely with the Program Manager to build and maintain an alliance of family support/home visitation providers for expecting parents and families with children birth to age five.In this role you will:Conduct phone screens with familiesMake referrals to home visitation programs and community resourcesMaintain information in a database and generate reports to partners and fundersFollow up with families and community programs regarding servicesKeep detailed records of phone interactions with familiesPlan and staff community outreach activities to recruit families for home visitation services in Maricopa CountyWhat it takes:Associate degree, Bachelor's degree preferred.Bilingual in Spanish required.One (1) year experience in community outreach, engagement, recruitment, and coordination with community providersKnowledgeable about home visitation and the home visitation systemSkilled in data collectionKnowledgeable of community resources in Maricopa CountyValid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card.Learn more.Our core values embody Southwest Human Development's commitment to services that uplift the lives of children, families and other care providers, and the communities in which they live.
