This position works as a member of the hospice interdisciplinary team to provide support and to implement the Plan of Care initiated by the RN.RequirementsPosition Qualifications:Candidates must have a current NYS LPN Licensure with 1-2 years of recent medical surgical experience employed as an LPN in an institutional setting, home care, or skilled nursing facilityGraduate of an Accredited School of NursingApplicant must demonstrate basic knowledge of nursing theory and practice, knowledge of appropriate technology, knowledge of practice standards for area of responsibility, sound clinical skills, sound standards of practice, and positive communication skills.Previous hospice/home care experience preferred.Must have a valid driver's license with a clean, acceptable driving recordReliable transportation requiredHospice is an equal opportunity employer.
