Responsibilities Work in outpatient Phase II, III, and IV Cardiac Rehabilitation Responsible for patient assessment, exercise prescription and cardiovascular risk factor modification education Proficient in the assessment and monitoring of patients, providing appropriate care and documentation, including patient condition, interventions, and communication with provider Ability to implement individualized exercise plans for patients physical and age specific needs and abilities including ensure that treatment plans are according to the AACVPR & ACSM guidelines Monitor patients for signs and symptoms during Cardiac rehab sessions Follow AACVPR guidelines Develop and lead a variety of patient centered exercises to include warm-ups, cool-downs and relaxation Conduct patient and family health education related to cad, risk factor identification and prevention develop and conduct outpatient and community presentations on health and wellness Recognize and take appropriate action to complications related to exercise such as arrhythmia detection, intervention and or chest pain / sob Perform any and all other duties as assigned QualificationsJob Requirements: Bachelor's Degree in Exercise Science or related field 1-3 years Cardiac Rehabilitation experience Current BLS certification Preferred Job Requirements: Master's Degree or equivalent Cardiac rehabilitation experience
