POSITION SUMMARY
Coordinate Psychiatrist's schedule and patients to ensure consistent and timely care. Administer nursing care to patients. May advise patients on health maintenance and provide case management.
ESSENTIAL FUNCTIONS
Essential Functions Statement(s)
- Welcome patient and confirm patient's statement of current problems and symptoms.
- Complete patient assessments; review problem list and update record; review and update patient medication; record medical information and vital signs; monitor, record, and report symptoms and changes in patients' conditions; and maintain accurate and detailed reports and records.
- Schedule patients for Psychiatry Clinic Visits and conduct reminder phone calls for scheduled visits.
- Manage and coordinate Psychiatrist's schedule by, prepping patient charts, organizing diagnostic and preventative reports for provider review, preparing needed equipment prior to patient's appointment, direct other staff as needed to ensure patients have been assessed and ready for provider visit.
- Facilitate and schedule follow-up testing and appointments such as lab, x-ray, additional clinic visits and complete referrals when required.
- Work with provider to manage patient prescriptions.
- Review new medications with patient, family and caregiver; remind patient to call with any questions.
- Answer phone calls and facilitate communication between patient and provider.
- Complete any prior authorization needed for visits.
- Prepares for, attends and participates in team meetings, quality improvement projects, team huddles and scheduling huddles with provider and registration.
- Supports and abides by the mission, values, and policies of CHCS in all activities.
- On-going compliance with CHCS educational requirements.
- Assist with providers in specialty clinic as needed.
- All other duties as required.
Requirements
RN or LPN Licensed in the State of Kansas
BLS certified
