MDS Cooordinator

Job Description

The incumbent will be responsible for the clinical and financial management of all Medicare A residents within Rice Estate. This will include pre-admission screening, appropriate assignment of residents within the RUGS IV hierarchy, coordination of therapy and nursing needs for each resident. In addition, the incumbent is responsible for coordinating the completion of the 5, 14, 30, 60, and 90 day assessments, any significant changes or OMRA assessments and the development of an interdisciplinary care plan based on those assessments. The incumbent also assists in the maintenance of appropriate nursing policies and procedures in order to comply with accepted nursing practice and federal, state, and local regulations. Will provide oversight of the day to day charting, ensuring quality documentation of daily skilled services provided. Will provide any in servicing and education necessary to enhance the knowledge and abilities of the staff to ensure quality care. The incumbent will develop and maintain a good working rapport with interdepartmental personnel as well as with individuals within the medical and hospital communities to ensure that appropriate services will be maintained and provided to meet to the needs of the residents.

Qualifications

• Must be an LPN or RN, licensed to practice in the State of South Carolina.
• Minimum of two (2) years nursing experience.
• Knowledgeable about the assessment of residents and the development and coordination of the interdisciplinary plan of care.
• Demonstrates the ability to work effectively and calmly during a crisis; able to make and execute decisions.
• Demonstrates the ability to communicate effectively, supervise and work well with other nurses and department heads.
