Job Summary:

The RN Case Manager for Utilization Management will coordinate all systems required for a multidisciplinary, patient-centered care team approach to ensure quality patient outcomes within established time frames and efficient resource utilization. As the contact person for patients, families, and healthcare team members, the RN Case Manager performs initial and ongoing assessments, manages treatment plans, ensures continuity of care through discharge planning, and analyzes variances. Responsibilities include:

  • Identification of appropriate patients for case management interventions by utilizing established procedures
  • Performing assessment of the patient's clinical, psychosocial status and treatment plans
  • Assessing patient/family/significant others’ needs and providing treatment options and discharge planning in collaboration with appropriate resources
  • Developing a comprehensive multidisciplinary plan of care
  • Coordinating and evaluating the use of resources and services in a quality-conscious, cost-effective manner
  • Monitoring and evaluating short-term and long-term patient responses to interventions
  • Ensuring adherence to Hospitals and departmental policies and procedures
  • Participating in continuous quality improvement activities

Qualifications:

  • Graduate of a nationally accredited nursing program, Bachelor's Degree preferred
  • Current RN license in NM or as allowed by reciprocal agreement by NM
  • CPR for Healthcare/BLS certification within 30 days of hire
  • 1 year of directly related experience

Physical Conditions:

This position is classified as light work, with physical demand requirements in excess of those for Sedentary Work. May require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or may require working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of materials is negligible.
