Monogram Health is seeking a board-certified Palliative Care physician in (Newark) New Jersey. Treatment and oversight will occur through telehealth and locally in person. The physician will connect with CKD and ESRD patients who are appropriate for conservative care. The physician will also engage with patients who have multiple complex co-morbidities. be expected to manage patients who are on our conservative pathway.
This job is ideal for physicians who embrace the trends of telehealth and value-based care, rewarding outcomes rather than utilization. The physician must have experience in end-of-life issues, advocating for conservative care where appropriate and where evidence-based criteria show that dialysis will not extend the duration of life. Monogram physicians will work remotely from home and locally in person when necessary. This is a full-time role, 5 days a week, Monday through Friday.
Responsibilities:
Build & Manage Panel:
Collaborate with POD Medical Director to proactively identify CKD& SRD members in predefined geographic regions to build patient panel
Own all outcomes of patient panel, driving clinical and financial KPIs
Drive Clinical Interventions for Patient Panel:
Treat panel of patients both in-home and virtually as necessary to drive healthcare outcomes
Apply evidence-based criteria in determining the appropriateness and necessity of care;
in particular, in determining whether dialytic or non-dialytic care is the correct evidence-based pathway
Reduce pain in patients who have selected a palliative care pathway
Round on patient panel (if admitted) in selected hospitals within a defined geographic market
Reduce preventable episodes of care including hospital readmissions
Refer members to appropriate specialists to drive clinical outcomes
Develop key relationships with local providers (PCP, Nephrologists, Specialists) to improve market alignment
Support Clinical Team:
Work with local Monogram PODs to drive KPIs
Participate in monthly sessions with the Regional Medical Director and CMO concerning KPI expectations
Backup coverage will be provided by Monogram MD team members
MD must maintain active privileges at local dialysis centers and hospitals
Review Annual Wellness Assessments to ensure proper coding and provision of medical diagnosis
Assist in the training of NPs and other providers
Requirements:
1+ years treating patients wiwth CKD and ESRD
Demonstrated experience applying evidence based clinical criteria
Experience in geriatrics or palliative care
Comfort and skill in end-of-life discussions
Experience with high-need Medicare Advantage and managed Medicaid populations
Expertise in managing medication panels for complex patients
Current state medical license
Willingness to become licensed in multiple states
Doctorate degree in medicine
Board-certified in geriatric medicine or hospice and palliative medicine
