Overview St. Luke's Health System in Meridian is seeking a Home Infusion Liaison to join our Home Infusion Pharmacy team. Monday-Friday Day Shift Responsibilities The Home Infusion Liaison serves as an expert clinical consultant to hospital staff and clinical educator. Advocates for patients with the goals of raising awareness about the benefits of home infusion and optimizing the transitions of care for patients referred to St. Luke's Home Infusion. In this role, you will: Educate patients, physicians, case managers, and customers regarding disease states, infusion therapy, transitional care management, and other St. Luke's Home Infusion programs and services at clinical sites throughout the health care system. Develop and support systems to facilitate timely placement of discharged patients requiring home infusion or enteral services by gathering preadmission data and collaborating with hospital discharge team including initial patient set-up/training and ongoing patient/caregiver education. Interface in-person and virtually with healthcare professionals and hospital staff in an effort to strengthen partnerships and grow business through referrals. Requirements Degree from an accredited pharmacy program Two (2) years' experience in a clinical role within a health system Transitions of Care experience preferred Robust patient education experience preferred Valid Idaho Pharmacist License Why St. Luke's St. Luke's Health System is Idaho's largest employer and most awarded health system, with a medical staff of over 1,800 physicians and advanced practice providers.
