Category:Admin SupportCity:JacksonState:TennesseeShift:0 - Day (United States of America)Job Description Summary:Performs analysis of the medical record of discharged inpatient, observation, outpatient and emergency room records in accordance with the Medical Staff Bylaws and Rules and Regulations. Assigns deficiencies for missing documentation, required dictation, and signatures. Performs reanalysis of declined and completed records by providers. Monitors delinquent medical records and performs the weekly suspension process as overseen by the manager. Utilizes Discern Reporting Portal to run reports. Performs HIM provider orientation for new providers. Assists medical staff providers with completion of their assigned deficiencies. Under the direct supervision of a Manager of Health Information Management responsible for assigned shift 5-7 days per week. This includes weekend rotation, and irregular hours. Employee is subject to call back and overtime as required by the hospital.ESSENTIAL JOB FUNCTIONS:Performs technical functions related to the initial analysis of the patient record. Evaluates the medical record for complete status and in accordance with Medical Staff Bylaws and Rules and Regulations. Electronically assigns deficiencies for chart completion.Performs secondary analysis on records that have been declined and or completed by providers. Reassigns deficiencies as appropriate. Follows up on each deficiency declined by provider until resolution is satisfactorily reached from both a physician and hospital standpoint. Completes the delinquency and suspension process as outlined in the Medical Staff Bylaws. Crosschecks dictated reports in the transcription system. In the absence of the manager, responsible for initiating all reports required for the completion of the suspension process, including distributing email notification of the suspended providers. Runs and works verbal order report. Utilizing the Discern Reporting Portal, runs daily reports - complete dictate report, and refusal report and completes. Utilizes the daily cardiology lists to assign stress test and Holter monitor deficiencies. Acts as liaison for providers with other areas of HIM, Help Desk, and other departments for problem resolution.Performs HIM provider orientation as it relates to message center, required deficiencies and navigation of the EMR. Provides excellent customer service to our providers; resolves requests and follows-up with the provider to meet their needs.Performs audits as assigned to document compliance of provider's documentation. Assists Coding Specialists with documentation issues such as needed deficiency assignment or query that is hindering coding the medical record.Manages faxes from outside clinics and physician offices for incomplete medical records.Meets all established productivity and quality standards. Performs other tasks as assigned or directed by a manager. Tasks are completed within the assigned timeframe.JOB SPECIFICATIONS: EDUCATION: Skill and proficiency in the principles, concepts, and techniques of Health Information Management, normally acquired through 5+ years experience in Health Information Management. An associate's degree or minimum of 5 years experience in Health Information Management: degree in Health Information Technology or Management preferred. LICENSURE, REGISTRATION, CERTIFICATION:Registration and certification as a RHIT or THIA by AHIMA preferred ,EXPERIENCE:Knowledge in Health Information Management practices, as normally acquired through RHIT or RHIA credential, or minimum 5 years previous experience. Experience with an electronic medical record preferred. KNOWLEDGE, SKILLS AND ABILITIES: Above average skills with Microsoft Office, particularly Excel spreadsheets required.
