Duties:
  • Works independently to provide coverage for a variety of tasks associated with a remote records room(s).
  • Routine Medical Record Department technical processing functions include: record retrieval and delivery, chart pick up, record processing for completion, assembly, analysis, transcription processing, physician assistance, microfilm processing, release of information, chart locator updates, chart release, chart deficiency, telephone coverage utilizing appropriate customer service skills, and filing.
  • Incumbent may perform only certain of the following responsibilities depending on their particular work assignment.
Responsibility A:
  • Process routine disclosures of Protected Health Information (PHI) to patients, families, insurance companies, attorneys, etc. according to federal/state laws and regulations and non-routine disclosure of PHI in response to court orders and subpoenas.
Responsibility B:
  • Retrieves, delivers, compiles, scans and/or processes medical records.
Responsibility C:
  • Analyzes medical records for accuracy.
  • Analyzes medical records for deficiencies in accordance with JCAHO standards, medical staff by-laws, rules and regulations for accuracy.
  • Follows the department analysis guidelines.
  • Interprets notes to determine whether there are deficiencies in medical records.
  • Determines appropriate physicians to complete deficiencies in the medical records.
  • Assist others in resolving problem charts.
  • Enters deficiencies into the Electronic Health Records.
  • Responsibility D: Processes STAT requests, walk-ins and transfers.
  • Responsibility E: Provides excellent customer service to patients, physicians, staff and all other customers.
  • Responsibility F: Other duties as assigned.
  • Knowledgeable in the use and analysis of HIM patient information systems applications.
  • Knowledge of medical terminology. Knowledge ofICD9, rCDIO, and CPT coding classification systems and guidelines.
  • Knowledgeable in CMS, Medicaid, and other regulatory agency rules and regulations related to Health Information Management.

Speaking:
  • Expressing oneself verbally in an accurate, understandable, and succinct manner to individuals or groups; using appropriate non-verbal signals to emphasize spoken words; adapting communication style and tone to fit the situation and audience; demonstrating poise during group discussions or formal presentations; holding others' attention; responding clearly and tactfully to questions.
