DescriptionCare Connector Job Purpose: The responsibility of the Care connector is to facilitate care between primary care, behavioral health, psychiatry, and case management teams regarding patient health care needs. This individual will record progress and status of internal/external referrals in the patient's chart and is accountable for the information flow of internal/external referrals between patients and the clinical/medical teamsMain Duties and Responsibilities:•Provide ongoing treatment coordination with behavioral health, primary care, psychiatry, and case management teams. o Initiate coaching, information, and support to empower patients to become an active participant in their treatment opportunities and planning.o Actively coordinate care and provide regular staffing/updates with the treatment teams. Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patient and their family.o Participate in regular team meetings, departmental, and organizational committees Demonstrate empathy and compassion in all patient interactions.•Create, update and access confidential patient information while adhering to HIPAA guidelines.o Verify patient information.o Perform routine data entry into various software systems.o Answer or return patient and provider inquiries.o Add information to appropriate software resource.o Schedule appointments within specified guidelinesOther duties as assigned.Requirements:HS Diploma (or GED) (Required); Bachelor's in related field (preferred)Qualified as a Behavioral Health Technician (required); a bachelor's degree in behavioral health, counseling, psychology, social work or another relevant field, or an associate's degree with at least one year of full-time behavioral health work experience, or a high school diploma (or equivalency) with at least three years of full-time behavioral health experienceValid AZ LPN or LVN license (preferred)Valid and current CPR, First Aid Certification, and an Arizona fingerprint clearance cardAbility to communicate effectively orally and in writing, demonstrating professional writing skillsExcellent communication skills virtually and in personBasic to intermediate knowledge of customer service protocols and patient engagement strategiesKnowledge of, and ability to use, electronic record keeping systems (EMR, billing software, insurance carrier portals, spreadsheets, etc.)Bilingual English/Spanish fluency preferredDenova Collaborative Health is an integrated primary care and behavioral health practice based in the Greater Phoenix metropolitan area.
