As our Healthcare Liaison, you'll be the linchpin in crafting smooth transitions for patients from rehab to home, working closely with a multidisciplinary team to tailor post-discharge plans. But that's not all. We're looking for a dynamic individual who can also wear the hat of a business developer - someone who can nurture and grow our partnerships with healthcare facilities through strategic relationship-building and effective communication.Why You're Perfect for This Role:You have a Valid LPN or RN license in New Jersey and 3+ years of experience in a nursing facility or similar healthcare environment.Your background in care management shines, especially in coordinating patient care post-discharge.Clinical skills? Top-notch.
