The Registered Nurse Case Manager is responsible for the overall direction of home health clinical services. The Registered Nurse Case Manager will have overall responsibility for the quality, adequacy of services provided and oversight of staff. This individual establishes, implements, and evaluates goals and objectives for home health services that meet and promote the standards of quality. We are looking to add an enthusiastic member to our independently owned and operated agency.

Responsibilities of the Registered Nurse Case Manager:

  • Effectively communicate with families, community partners, and other healthcare providers to ensure adequacy of services provided and supervision of staff.
  • Makes the initial evaluation visit and regularly reevaluates the client’s caregiver and nursing needs.
  • Review physician’s orders and update the plans of care and documentation as necessary.
  • Provides care as defined within the scope of the state’s nurse practice act.
  • Supervises, educates and coaches other nursing personnel/care-giving staff.
  • Plans and implements in-service and training programs.
  • Assists with the organizational goal to grow infusion and skilled services business.
  • Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  • Develops, recommends, and administers agency policies and procedures.
  • Prepares clinical and progress notes and summaries of care.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
  • Promotes home health referrals in the health care community.

Requirements of the Registered Nurse Case Manager:

  • Current unrestricted RN license in PA
  • Excellent customer service skills and ability to deal tactfully with clients, staff, and community.
  • Treat clients, staff, and the public with courtesy and respect, and present a positive public image.
  • Preferred: Bachelor’s degree in Nursing. Must have graduated from a National League of Nursing accredited program.
  • Clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred.
  • Infusion experience preferred
  • Reliable car/auto insurance
  • Clean background and criminal record
  • Understands regulatory compliance with state and federal regulations
  • Adhere to HIPPA and maintain client confidentiality

Working Conditions:

  • Travel includes various sales, marketing and client care service needs and attending conferences and planning meetings.
  • Contact with clients under a wide variety of circumstances.
  • May be required to respond to emergency situations.
