With our combined resources CommonSpirit is committed to building healthy communities advocating for those who are poor and vulnerable and innovating how and where healing can happen both inside our hospitals and out in the community.**Responsibilities****Job Summary / Purpose**Provide safe, appropriate, and effective use of drugs in and outside the hospital by serving as a resource person and a clinical liaison to physicians, pharmacists, nurses, and other hospital personnel. Perform duties of a staff pharmacist as necessary. Provide care to patients of all ages. Provide patient care/services based on identified needs/developmental level for all patient populations checked below; care/services may include assessment of individual patient need; incorporation of age-specific characteristics in patient education, communication, and planning; provision of individualized care relevant to patient needs; and recognition, monitoring, and provision of interventions as appropriate.**Essential Key Job Responsibilities**1. Complies with Hospital/Department/Unit policies.2. Documents activities consistent with Hospital/Department/Unit Guidelines.3. Exhibits mastery of clinical initiatives, pharmacokinetics, anticoagulation, and nutrition. Exhibits competency in clinical microbiology and therapeutic areas consistent with practice.4. Actively monitors drug therapy and provides pharmacist clinical interventions according to assignment and as identified by patient needs.5. Serves as Chief Pharmacy Resident as directed by the Residency Program Director. Coordinates residency activities as requested.6. Participates in medication error avoidance programs and routinely reports medication errors and adverse events.7. Works with Clinical Pharmacy Manager to coordinate student orientation and rotation activities. Facilitates student assignments and prepares a monthly calendar for students and preceptors.8. Completes primary resident project and adheres to research timeline. Provides drug review, MUEs, and other projects as requested by Residency Program Director.9. Mentors pharmacy students during clinical rotations as assigned by Residency Program Director and residency preceptors. Provides appropriate clinical and teaching interactions.10. Actively participates regularly with Clinical Conference. Presents to the group as requested. Disseminates information with pharmacy staff as appropriate and as requested.11. Provides operational services per PGY1 policy. Maintains the operation of the unit dose area, IV room, and satellite pharmacy effectively.12. Demonstrates competency in maintaining computerized medication profiles and processing medication lists for cart filling and IV admixtures as well as dispensing medications (including controlled substances) according to procedures and state and federal laws.13. Performs direct patient care activities in Anticoagulation Clinic.+ Provides direct patient centered care to assigned patients. Actively monitors assigned patients in inpatient and outpatient areas to assure all patient care needs are met and intervenes when deficiencies are observed.+ Other duties as assigned._The job summary and responsibilities listed above are designed to indicate the general nature of the work performed within this job.
