All the relevant skills, qualifications and experience that a successful applicant will need are listed in the following description.Varies, Minnesota Kidney Specialists of MN (KSM) is hiring for an exciting opportunity as a dietician for our clinics KSM is an independent nephrology practice with 23 physicians and 8 Advanced Practice Practitioners delivering quality, patient-centered care in the twin cities and outlying suburbs. KSM has been providing high-quality care to patients with CKD, diabetic kidney disease, and ESRD since 1978.
Job Purpose
* Provide Medical Nutrition Therapy (MNT) to patients who are referred by our KSM providers.
Job Description
* Work closely with the KSM Physicians and Team under a growing value based healthcare framework with the overarching goal of value for patients as it pertains to better patient outcomes.
* Review daily schedules and review patient charts prior to clinic appointment.
* Travel to assigned clinics to provide MNT to patients.
* Complete a nutrition assessment, determine priorities for nutrition education/care and educate/provide nutrition counseling/coordinate nutrition care as indicated. Provide appropriate written education materials.
* Provide education support to providers and staff as requested.
* Obtain or develop education materials as needed.
Skill Requirements/ Qualifications
* Requires Registered Dietitian or Registered Dietitian Nutritionist (RD/RDN) credential AND state licensure credential of Licensed Dietitian or Licensed Dietitian Nutritionist (LD/LDN)
* Requires knowledge of nutrition care for people with Chronic Kidney Disease and associated diseases
* Maintain up-to-date knowledge base in renal nutrition and in Medicare changes that affect MNT coverage.
