RN Case Manager FT

Responsible and accountable for case by case clinical, quality, and financial outcomes for patients in their caseload. Activities include concurrent clinical management, discharge planning, social work and utilization review.
REQUIREMENTS:
Minimum of 2 years clinical experience. Ability to manage conflict and negotiate during difficult situations. Self motivated. Must be able to set priorities at all times. Must be very detail oriented and follow-up on everything. Must be able to work closely with physicians, nurses and other ancillary departments. Abides by “Quality Expectations” with “Service Oriented” attitude. Must demonstrate professional level of conduct, appearance and attendance.
