**Job Summary**

Performs all functions of a nutritionist including conducting nutritional assessments, providing nutrition counseling to patients of the Health Center, preparing nutrition care plans, and serving as a nutrition resource person for departmental staff. Educates and counsels high-risk clients on an individual basis taking into consideration medical and nutritional needs, laboratory results, cultural and personal preferences, finances, and clients goals. Joins in organizational community outreach efforts by educating on good nutrition practices and its impact on peoples health.

**Essential Functions :**

- Assess and counsel patients on nutrition related health issues

- Creates nutrition care plan

- Complete WIC forms for WIC eligible patients

- Provide nutrition education in group visit setting for various patient groups (for example OB, pts with DM, or refugees)

- Participate in community events, health fairs as requested

- Assess and counsel students at SBHCs on nutrition related health issues

- Participate in joint visits with clinical pharmacy and CCMs as determined by patient needs

- Complete notes in EHR in timely fashion

**Education and Experience**

* Bachelors degree in Food and Nutrition or related field.

* Licensed Dietitian/Nutritionist or Registered Dietition.

* Bilingual in English/Spanish, English/Portuguese.

* Substitutions Allowed: Bilingual in English and one other language deemed by Health Center as an essential need.

* One-year experience required, preferably in a public health setting or in a community nutrition program.
