Job Summary

A registered licensed nurse designated to act as the coordinator of the center’s wound team to oversee the

general application of the skin management program. The following activities are performed as directed or

clinically indicated.

Residents Rights

• Knows and respects resident rights

• Reports complaints made by residents to supervisor

• Reports allegations of resident abuse, neglect and/or misappropriation of resident property

Safety and Sanitation

• Follows established safety policies and procedures Observes safety needs of residents as indicated in

care plan

• Wears and/or uses safety equipment and supplies when indicated and properly trained to use

• Demonstrates job-specific knowledge of fire and disaster preparedness during drills or actual situations

Staff Development

• Attends and participates in scheduled in-service training, educational classes and meetings to

maintain current certification as applicable and as managed by regulatory agencies and company

standards

• Participates in QAA program

Wound Care Nurse Responsibilities

• Identify and maintain effective methods to gather pressure ulcer and skin alteration related information

• Coordinate and lead weekly wound team rounds and processes, including rounds, measurements,

interdisciplinary care plan review, treatment recommendations, physician orders, and required

documentation

• Collect, analyze and provide pressure ulcer information to nursing staff and other healthcare

practitioners

• Investigate and identify center practices and risk minimizing measures, including accurate completion

of Braden risk scales on admission, implementation of preventive interventions

• Monitor timely implementation of preventive interventions for at risk residents, including new

admissions, individuals with skin alterations and individuals at risk for development of skin alterations

• Maintain records of pressure ulcers and skin alterations including risk assessment, prevention and

minimizing activities

• Coordinate completion of ongoing pressure ulcer and skin alteration tracking activities and implements

measures to improve outcomes, including employee education

• Review pressure ulcer rates monthly providing recommendations to the Quality Assurance and

Performance Improvement Committee

• Recommend education for employees and other healthcare practitioners to minimize and, or prevent

center acquired skin alterations and pressure ulcers

• Educate newly hired nursing staff on wound assessments, preventive interventions, care planning,

treatment selection options, documentation requirements and education resources available

• Collaborate with administrator, medical director or his/her designee, Director of Nursing and other

members of health care team on skin practice.

• Evaluate availability of needed supplies for wound care and coordinate ordering with central supply

• Assist the Director of Nursing and medical director with case review for center acquired pressure

ulcers

• Monitor changes in state and federal regulations and current industry-standard practices

• Provide monthly, quarterly and annual reports including recommendations for changes in center

practice for the Quality Assurance and Performance Improvement Committee

• Keeps current with industry trends through ongoing specialized education in the area of skin and

wound management.

General Nursing Care Responsibilities

• Demonstrates knowledge and appropriate use of the (Center) Infection Control Manual.

• Understands, participates and assists in implementing the Quality Improvement Program.

• Notifies other Departments of resident changes, i.e., room changes, LOA, new admits, discharge, etc.

• Conducts family and resident education.

• Demonstrates the ability to administer medications timely and according to center policy.

• Demonstrates the ability to administer treatments timely and according to center policy.

• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice

Act

• Makes rounds with physicians when necessary.

• Transcribes physician’s orders to resident charts, cardex, medication/treatment cards and care plans, as

required.

• Charts progress notes in an informative, factual manner that reflects the care administered as well as

the resident’s response to care.

• Follows established procedure for charting and reporting all reports of incident/accidents (residents &

employees).

• Identifies and reports changes in condition to supervisor, physician and family.

• Accurately identifies skin changes and follows skin management protocols.

• Orders and reports diagnostic tests timely.

• Conducts frequent rounds to evaluate effectiveness of care delivery.

• Orders supplies and charges for items ordered through Central Supply.

• Follows established procedures for restraint reduction.

• Follows center policy and procedures in the event a restraint must be utilized.

• Assures that an adequate supply of personal protective equipment is on hand and are

available for staff to perform procedures that involve exposure to body/blood fluids.

• Counts narcotics at the beginning and completion of each shift with the nurse leaving and coming on to

the shift.

• Follows Pharmacy policy and procedures for ordering and delivering medications.

• Coordinates resident’s rehabilitation program with therapies to ensure 24 hour rehabilitation needs are

met.

• Plans nursing interventions for each resident based on their individual needs and monitor the

implementation.

Special Nursing Care Responsibilities-Based on State Nurse Practice Act

• NG/tube: insertion, placement, irrigation

• Colostomy care: irrigation

• Foley catheter care: insertion, Irrigation

• Texas catheter application

Education

• Currently licensed as LPN/RN in state
