Shared Support South, Inc is currently seeking for dedicated LPNs to work in our home base setting with an individual that are Trach, G- Tube and Vent dependent.

$38 PER HOUR

Qualifications

Role and Responsibilities

• Ensure the well-being of the consumer.
• Provide appropriate care in accordance with personal centered plan and all physician order.
• Provide home care nursing to adult consumer in home and community setting.
• Create a self-centered environment for the individual - focused on his/her hopes, dreams, and goals.
• Follow all care plans, ISP, and protocols.
• Ensure that individuals home and medical equipment is clean and safe.
• Lead for insuring individuals health maintenance
  
  

Required Skills

• Excellent Documentation skills
• Proficient in G-Tube and J-Tube care and medication administration
• Ventilator experience
• Tracheotomy experience
• Use of appropriate policies and systems as required for this position.
• Compassionate, caring, and punctual
  
  

Qualifications And Education Requirements

• Current unencumbered nursing license (LPN) in state of PA
• Current CPR
• Minimum 1 year LPN homecare experience
• Minimum 1 year Trach, Vent, and G Tube experience
• Graduate of an accredited nursing program
