VA San Diego Healthcare System 3350 La Jolla Village Dr., San Diego, CA 92161

Locations:

  • Clinic locations include VASDHS, Kearny Mesa VA Clinic, Oceanside VA Clinic, Chula Vista VA Clinic, and Sorrento Valley VA Clinic.

Work Schedule:

Monday through Friday between the hours of 8:00 a.m. and 4:30 p.m.

Objectives:

As an Internal Medicine Physician at Sanford Federal, Inc. you will play a vital role in delivering primary care services to veterans at multiple clinic locations within the VA San Diego Healthcare System. You will work in a patient-centered care environment as part of a collaborative team, adhering to the Patient Aligned Care Team (PACT) model.

SANFORD Responsibilities:

Patient-Centered Care:

  • Engaging patients and families in self-management and goal setting.
  • Providing education tailored to patients' care needs.
  • Encouraging patients to enroll in MyHealtheVet and Secure Messaging.
  • Ensuring timely communication of test results to patients.
  • Delivering comprehensive primary care, including acute and chronic illness management, mental health screening and treatment, preventative care, and gender-specific care.

Access to Care:
  • Offering same-day access for patients.
  • Conducting face-to-face and virtual visits.
  • Providing telehealth services to enhance patient access.
  • Care Management & Coordination:
  • Managing transitions of care from inpatient to outpatient settings.
  • Collaborating with specialty care providers.
  • Developing community and VA interfaces for patient care.
  • Managing patient populations, especially those with chronic illnesses.
  • Focusing on health promotion and disease prevention.
  • High-Risk Patient Management:
  • Identifying and managing high-risk veterans with complex medical needs.
  • Utilizing face-to-face, telehealth, and virtual care delivery methods.
  • Practice Design & Flow:
  • Maximizing team efficiency through clear role and task clarification.
  • Ensuring team members work to their maximum ability, skill, and license.
  • Continuous improvement of workflow processes.
Qualifications:
  • Medical degree with a current, unrestricted license to practice medicine in any state, territory, or commonwealth of the United States or the District of Columbia.
  • Board Certification/Board Eligibility in Family Medicine.
  • VHA-approved Basic Life Support (BLS) certification.
  • Compliance with all VA policies, directives, and handbooks related to patient care and provider responsibilities.
  • Strong communication skills and the ability to work effectively in a team-based environment.
  • Commitment to providing high-quality care to veterans.
