If that sounds like an organization you want to be a part of, we would love to have you.ROLE OVERVIEW and PURPOSEThe Outreach Specialist will conduct outreach in the community to promote and raise awareness about the Neighborhood Healthcare PACE program. The Outreach Specialist will focus on generating leads from multiple referral sources resulting in PACE program enrollments.Additionally, this position will collaborate with the Enrollment Specialists team to increase census within the program.RESPONSIBILITIESConducts targeted outreach and training to community-based organizationsDiscovers and participates in community events and fairs to build relationships and partnerships with other community organizations, such as food pantries, churches, and othersWorks collaboratively with the Outreach and Enrollment supervisor to develop outreach strategies and to foster relationships within the communityMaintains reports to assess and analyze outreach activities to maximize enrollmentsDevelops rapport and maintains relationships with other departments to bring awareness regarding other services available within the communityProvides exceptional customer service to potential participants for our PACE programs and servicesPrepares written reports and maintain accurate records, including outreach logsProvides program tours, intakes, education, and enrollment assistance to determine participant needs and enrollment eligibility, including in their participants homes, as neededEDUCATION/EXPERIENCEHigh school/GED required; bachelor's degree in marketing, communications, public relations or related field preferredValid CA DHCS PACE and Medi-Cal Marketing certification or must obtain within 60 days of hireTwo years' customer service experience required; sales experience preferredExperience working in healthcare and marketing to underserved populations preferredExperience with HMO, Medi-cal/Medicaid, and senior care related sales and marketing preferredValid CA driver's license and proof of auto insurance requiredADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with Neighborhood Healthcare and PACE programs and servicesKnowledgeable about and experience with sales and marketing techniques to outreach to diverse social and ethnic groups and to communicate effectively with them and the ability generate referralsKnowledgeable about and experience with systems and procedures to efficiently track and trend all sales activityKnowledgeable about and experience with create contacts and generate a referral sourceKnowledgeable about and experience with Principles and practices of health care service delivery, managed care, health care systemsAbility to successfully manage multiple tasks simultaneouslyExcellent planning, presenting, and organizational abilityAbility to cultivate and build positive relationships with internal departments and external stakeholdersAbility to apply strategic and analytical thinkingAbility to work as part of a team as well as independentlyAbility to work flexible schedules, including weekendsAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 15 lbs/weightAbility to stand for long periods of timeNeighborhood Healthcare offers a generous benefit plan that includes:Partially company paid Medical, Dental, and Vision Plans. Two plus weeks of vacation, Nine Holidays including two Floating Holidays of your choosing, Sick/Personal time, Volunteer Time Off (VTO), 403b Retirement plan (similar to a 401k), optional Health and Wellness events, and much morePay range: $24.83 - $30.00 per hour, depending on experience.
