OverviewWe are seeking a dedicated and compassionate ACT Clinician to join our mental health services team. The Assertive Community Treatment (ACT) Clinician will play a crucial role in providing comprehensive, community-based mental health services to individuals with severe and persistent mental illness, particularly those experiencing homelessness with high recidivism rates. This position requires a dynamic individual who is passionate about delivering high-quality, person-centered care to promote recovery, stability, and improved quality of life for our clients. Essential Duties and FunctionsClient Assessment and Treatment PlanningConduct thorough assessments of clients' mental health, social, and functional needs, with a focus on addressing the unique challenges faced by individuals experiencing homelessness and high recidivism.Collaborate with clients, their families, and interdisciplinary team members to develop individualized treatment plans tailored to promote stability and community integration.Direct Clinical ServicesProvide individual and group therapy sessions using various therapeutic modalities, recognizing and addressing the trauma and systemic challenges often experienced by individuals living in homelessness.Offer crisis intervention services and facilitate timely access to psychiatric care as needed, acknowledging the heightened vulnerability of clients in this population.Participate in a compensated on-call rotation with other team members.Case Management and Support ServicesCoordinate with community resources, including housing, vocational rehabilitation, and substance abuse treatment programs, to address the complex needs of clients experiencing homelessness and high recidivism.Assist clients in accessing entitlements, benefits, and other supportive services essential for stability and recovery.Advocate for clients' rights and facilitate their integration into the community while promoting self-sufficiency and independence, recognizing the barriers and stigma faced by those with a history of recidivism.Multidisciplinary CollaborationParticipate actively in interdisciplinary team meetings to discuss clients' progress, treatment strategies, and ongoing support needs, with an emphasis on addressing the systemic factors contributing to homelessness and recidivism.Collaborate with psychiatrists, nurses, social workers, and other mental health professionals to ensure coordinated and comprehensive care delivery, advocating for trauma-informed and recovery-oriented approaches.Documentation and ComplianceMaintain accurate and up-to-date electronic health records, treatment plans, progress notes, and other required documentation in compliance with regulatory standards and organizational policies.Adhere to ethical guidelines and confidentiality protocols in all interactions with clients and team members, recognizing the sensitivity of information related to homelessness and recidivism.Other duties as assigned. RequirementsQualificationsMaster's degree in Social Work, Counseling, Psychology, or related field from an accredited institution.Licensure or eligibility for licensure (e.g., LCSW, LPC, LMFT) in the state of practice.Minimum of 2 years of clinical experience working with individuals with severe mental illness, preferably in a community-based setting.Strong knowledge of mental health diagnoses, treatment modalities, and recovery-oriented practices.Excellent communication, interpersonal, and problem-solving skills.Ability to work independently as well as collaboratively within a multidisciplinary team.Proficiency in using electronic health record systems and Microsoft Office applications.Valid driver's license and access to reliable transportation.Resources for Human Development is an equal opportunity employer.
